Introduction: Heart failure is a chronic progressive syndrome that can affect the quality of life of patients. Palliative care, as a holistic, effective approach, is significant in symptom management and health promotion.  This study aimed to determine the effect of palliative care on patients with heart failure.
Methods: This quasi-experimental study was conducted in 2025, at Shahid Rajaei Cardiovascular Institute, Tehran, on 56 eligible participants divided into (n=28) intervention and (n=28) control groups. After obtaining an Ethic Code and informed consent, the intervention group received 7 sessions of PC programs in addition to routine care via WhatsApp from discharge of hospital until one month follow-up visit. Data were collected using demographic questionnaires, Health Promoting Life Style Profile-II (HPLP-II), and Palliative Care Outcome Scale (POS). Data were analyzed by descriptive statistics, including independent t-tests, paired t-tests, Kolmogorov-Smirnov, and Levin test using SPSS 27 software.
Results: The groups were homogeneous in terms of demographic variables and questionnaire scores in the pre-test (P>0.05). The total score of HPLP significantly improved in the intervention group after receiving the palliative care program (P= 0.048). However, the subscale scores didn't change significantly.  The total score of POS in the intervention group was significantly lower (better) as well (P=004). The change in scores in the control group wasn't statistically significant (P>0.05).
Conclusion: Palliative care has a positive effect on improving health-promoting behaviors in heart failure patients, and it could be highly recommended as an effective care model in the country's protocols of cardiac nursing and treatment.
