fa بررسی تأثیر برنامه توانمندسازی بر آگاهی و عملکرد مادران کودکان مبتلا به تالاسمی ماژور The Effect of Empowerment Program on the Knowledge and Performance of Mothers of Children with Thalassemia Major سایر موضوعات Other topics پژوهشي Research مقدمه:  تالاسمی ماژور از شایع‌ترین بیماری‌های ژنتیکی در انسان است که مشکلات زیادی برای بیمار، خانواده و نظام سلامت ایجاد می‌کند. این مطالعه با هدف تعیین تأثیر برنامه توانمندسازی بر آگاهی و عملکرد خانواده کودکان مبتلا به تالاسمی ماژور مراجعه کننده به مرکز بیماران خاص شهرستان بیرجند در سال 1395 انجام شد. روش کار: در این مطالعه از نوع نیمه‌تجربی، طرح پیش‌آزمون- پس‌آزمون با گروه کنترل، 60 نفر از مادران کودکان دارای سن زیر 15 سال مبتلا به تالاسمی ماژور مراجعه کننده به مرکز بیماران خاص شهرستان بیرجند در سال 1395 که معیارهای در نظر گرفته برای ورود به مطالعه را دارا بودند، به روش نمونه‌گیری در دسترس انتخاب و به طور تصادفی در دو گروه آزمایش (30 نفر) و کنترل (30 نفر) قرار گرفتند. برای مادران گروه آزمایش، مداخله‌ای شامل اجرای الگوی توانمندسازی بر اساس گام‌های معرفی شده در الگو، در 5-3 جلسه به صورت بحث گروهی و نیز با استفاده از پمفلت و اسلایدهایی در مورد بیماری اجرا شد و مادران گروه کنترل هیچگونه مداخله‌ای دریافت نکردند. جهت جمع‌آوری داده‌ها از پرسشنامه‌های آگاهی و عملکرد محقق ساخته استفاده شد که قبل و بعد از مداخله توسط مادران هر دو گروه تکمیل شد. داده‌ها با استفاده از نرم‌افزار آماری SPSS 22 و آزمون‌های آماری خی دو، تست دقیق فیشر، t مستقل، t زوجی، من‌ویتنی‌یو، ویلکاکسون در سطح معنی‌داری 05/0p≤ تجزیه و تحلیل شدند. یافته‌ها: نتایج نشان داد که میانگین نمره آگاهی و عملکرد قبل از مداخله در مادران دو گروه آزمایش و کنترل تفاوت معناداری نداشت (0/05p>) ولی بعد از مداخله در مادران گروه آزمایش نسبت به کنترل به طور معناداری بیشتر بود (0/01p<). در گروه آزمایش، میانگین نمره آگاهی و عملکرد بعد از مداخله نسبت به قبل از آن به طور معناداری افزایش داشت (0/05p<) ولی در گروه کنترل، تفاوت معناداری در نمره قبل و بعد از مداخله مشاهده نشد (0/05p>). میانگین تغییرات نمره آگاهی و عملکرد در مادران گروه آزمایش نسبت به کنترل به طور معناداری بیشتر بود (0/01p<). نتیجه‌گیری: با توجه به نتایج به دست آمده می‌توان بیان داشت که اجرای الگوی توانمندسازی یکی از مهمترین رویکردهای مناسب جهت افزایش آگاهی و عملکرد مادران کودکان مبتلا به تالاسمی ماژور می‌باشد و موجب مراقبت بهتر و کارآمدتر و بهبود کلی زندگی این بیماران می‌گردد.   Introduction: Thalassemia major is one of the most common genetic diseases in humans that causes many problems for the patient, family and health system. The aim of this study was to determine the effect of empowerment program on the knowledge and family performance of children with thalassemia major referred to the Special Patients Center of Birjand city (Iran) in 2016. Methods: In this quasi-experimental study with a pre-test-post-test design with a control group, 60 mothers of children under 15 years of age with thalassemia major referred to the Special Patients Center of Birjand in 2016, which met the criteria for inclusion were selected by available sampling method and then randomly divided into experimental (n = 30) and control (n = 30) groups. For the mothers of the experimental group, an intervention including the implementation of the empowerment model based on the steps introduced in the model was performed in 3-5 sessions as a group discussion and also using pamphlets and slides about the disease and the mothers of the control group did not receive any intervention. Researcher-made knowledge and practice questionnaires were used to collect data, which were completed by mothers of both groups before and after the intervention. Data were analyzed using SPSS 22 software and Chi-square, Fisher's exact test, independent t-test, paired t-test, Mann-Whitney, Wilcoxon at a significant level of p≤0.05. Results: The results demonstrated that the mean score of knowledge and practice before the intervention in mothers of the experimental and control groups were not significantly different (p> 0.05). But was higher after the intervention in the mothers in the experimental group than the control group (p <0.01). In the experimental group, the mean score of knowledge and practice after the intervention increased significantly compared to before of intervention (p <0.05). But in the control group, no significant difference was observed in the score before and after the intervention. The mean changes in knowledge and performance scores in mothers in the experimental group were significantly higher than controls (p <0.01).
