Introduction: Running is one of the most popular forms of physical activity worldwide; however, its repetitive mechanical loading is associated with a substantial risk of lower-extremity musculoskeletal injury. This systematic review evaluated the current evidence regarding the effects of foot orthoses on injury risk factors in runners.
Methods: PubMed, Web of Science, Google Scholar, and PMC were searched for English- and Persian-language articles published from 2000 to June 2026. The initial search yielded 998 records. After removing duplicates and screening titles, abstracts, and full texts in three stages, 987 records were excluded, leaving 11 eligible studies for the final review.
Results: Among the reviewed studies, 72 percent examined the effects of custom orthoses, showing that their use can correct foot motion and reduce abnormal pressures. Also eighteen percent of the articles focused on specialized footwear and found that these shoes reduce fatigue and improve shock absorption. The remaining studies (10 percent) investigated combined interventions such as orthoses with anti-pronation taping or shoes with special soles, which resulted in greater foot stability and increased comfort while running. These results indicate that the use of orthoses and specialized footwear can make running safer and reduce the likelihood of injury for both the general population and professional runners.
Conclusion: The review of the conducted studies showed that custom orthoses and anti-pronation shoes effectively improve biomechanical parameters related to injury, reduce pain, and increase runners’ comfort. At the same time, the use of combined interventions provides more sustained protective effects on foot stability and force distribution.
