Introduction: Anxious adolescents face various challenges, particularly in the areas of attachment and mind wandering. This study aimed to determine the effectiveness of adolescent-centered mindfulness therapy on attachment-related difficulties and mind wandering in anxious adolescents.
Methods: This quasi-experimental study employed a pretest-posttest design with a control group. The study population consisted of anxious adolescent girls referred to counseling and psychological services in Ahvaz during the winter of 2025. A total of 40 participants (20 per group) were selected through purposive sampling and randomly assigned to two equal groups using simple lottery-based allocation. The experimental group received ten 60-minute sessions of adolescent-centered mindfulness therapy (three sessions per week), while the control group remained on a waiting list. Data were collected using a demographic information form, an anxiety inventory, the Vulnerable Attachment Style Questionnaire, and a mind-wandering questionnaire. Data were analyzed using the chi-square test and multivariate analysis of covariance (MANCOVA) in SPSS version 25.
Results: No statistically significant differences were observed between the experimental and control groups in terms of age range or parental education level (P > 0.05). Adolescent-centered mindfulness therapy significantly reduced attachment-related difficulties and mind wandering among anxious adolescents (P < 0.001).

Conclusions: Adolescent-centered mindfulness therapy may be used alongside other therapeutic approaches by healthcare professionals and therapists to reduce attachment-related difficulties and mind wandering in anxious adolescents.
